Rehabilitation Centers That Provide Post-Acute Care

Rehabilitation and nursing centers that provide post-acute care assist patients to regain their health and return to their own home. Patients who have been in the hospital due to an accident, illness or surgical procedure transfer to post-acute facilities as a bridge between the hospital and the patient’s recovery. Physicians who utilize post-acute care post discharge from the hospital are less likely to have readmissions to the hospital. Our providers establish short term rehabilitation goals for patients prior to release and transfer.  Positive outcomes are derived from positive communication between our providers in transitional care.
